The Section of Hematology at Dartmouth Hitchcock Medical Center in Lebanon, NH is seeking an Inpatient Advanced Practice Provider to support its program in hematology and hematologic malignancies.

The program serves a diverse patient population, providing a range of treatment options including stem cell transplantation, BiTE, and CAR-T therapy. This service is 24/7, and we are hiring across multiple (12hr) shifts (daytime, nocturnist, and mixed schedule opportunities are available).

Advanced practice providers work closely with physicians in a shared practice model. APPs provide care for patients throughout the course of their illness and are involved in initial treatment planning and counseling, assessments, supportive care during chemotherapy treatment, and surveillance and follow-up care after completion of treatment. In addition to working collaboratively with physicians, they play an integral role in coordinating care with other subspecialties, including radiation oncology and surgical oncology. The cancer center includes an active cancer clinical trials program and provides a full range of supportive services, including social work, palliative care, nutrition support, genetic counseling, and patient navigation.

This is an excellent opportunity for the right candidate to join a collaborative and supportive APP/MD team practice that provides care for patients with hematologic diagnoses who are admitted to the hospital due to complications of their malignancy, complications of their treatment, for palliative care, or for chemotherapy. Common cancer diagnoses are lymphoma, leukemia, and MDS. In addition to treating patients with chemotherapy or on clinical trials, the team manages acute medical problems such as neutropenic fever, nausea and vomiting, pain management, and other acute medical diagnoses. This position offers the opportunity to be involved in every aspect of cancer care including initial diagnosis, treatment, clinical trials, management of complications/acute medical issues, and palliative care.

The position is 100% clinical. The typical morning consists of pre-rounding on patients followed by rounds with the attending physician. In the afternoon, the APP carries out the plan of care for patients, consults with other services as indicated, admits and discharges patients, and performs procedures (including bone marrow biopsy). The ideal candidate is motivated, self-directed, compassionate, enthusiastic and committed to evidence-based practice and lifelong learning. There are many opportunities for learning including teaching on rounds, weekly conferences, monthly staff meetings, and clinical care conferences.

Required Qualifications:
• Nurse Practitioner with a Masters of Nursing from an accredited program with current NH license, or eligible for licensure in NH.

OR
• Current (or eligible) certification to practice as a Physician's Assistant in the state of New Hampshire. Academic preparation at the Master's level.

Preferred Qualifications:
• For NP candidates: Advanced Oncology Nurse or Nurse Practitioner Certification (AOCN or AOCNP)
• For PA candidates: CAQ in Hospital Medicine (Physician Assistant candidates)
• Hematology/Oncology experience
• Inpatient Internal Medicine experience

Responsibilities:
• Complete/comprehensive history and physical examination
• Ordering and interpretation of diagnostic studies
• Prescribing appropriate treatments in collaboration with the attending physician
• Consultation with specialist physicians and other team members regarding findings/changes in patient condition
• Initiation and adjustment of chemotherapy orders in collaboration with the physician
• Documentation of patient care/procedures per policy
• Ensuring longitudinal communications with primary hematologist/oncologist and referring providers regarding patient status
